LIABILITY WAIVER & RELEASE

PLEASE READ CAREFULLY. THIS DOCUMENT RELEASES LAUNCH LABS LLC FROM LEGAL LIABILITY.

1. Assumption of Risk

I acknowledge that use of the golf simulators and facility at 565 W Oates #100 Rd involves inherent risks, including but not limited to: being struck by a golf ball or club, ricochets from screens or enclosures, trips/falls on hitting mats, and physical overexertion. I voluntarily assume all known and unknown risks of injury.

2. Release and Waiver of Liability

To the maximum extent permitted by Texas law, I hereby release, waive, and discharge Launch Labs LLC, its owners, and employees from any and all claims for personal injury, death, or property damage, INCLUDING CLAIMS CAUSED BY THE NEGLIGENCE OF LAUNCH LABS LLC, arising out of my use of the facility.

3. 24/7 Self-Service & Surveillance

I understand Launch Labs is an unstaffed, 24/7 self-service facility. I acknowledge that there is no medical or safety supervision provided. I consent to 24/7 video recording of my activities for security and insurance purposes.

4. Indemnification

I agree to indemnify and hold harmless Launch Labs LLC against any and all claims, suits, or actions of any kind brought by me or my guests (including minors under my supervision) for liability, damages, or costs, including attorney fees.

5. Equipment Damage

I accept full financial responsibility for any damage to the simulator technology, impact screens, or building property caused by my (or my guests’) misuse or failure to follow facility rules.
